Happy Hearts: BP & Mood Care

Happy Hearts is a Medicare-enrolled health care provider participating in the CMS ACCESS Model. Happy Hearts develops and provides care for the qualifying conditions in your ACCESS clinical track and coordinates with your doctor or care team. Depending on your ACCESS clinical track, care may include high blood pressure, high cholesterol, obesity or certain overweight conditions, prediabetes, diabetes, chronic kidney disease, heart disease, anxiety, or depression.

If you're eligible and have Original Medicare, there's no cost to you.
WHAT YOU CAN DO

For blood pressure, cholesterol, weight, or blood sugar conditions:
- Check your blood pressure with a monitor made for your upper arm. No setup calls.
- Happy Hearts explains what each reading means, in plain words.
- See your trends over time, against your own baseline.

For anxiety or depression:
- Answer a short weekly check-in about how you've been feeling.
- See your own patterns over time, in plain words.

For every enrolled member:
- Get medication reminders, set the way you want them.
- We send care updates to your doctor or care team — with your consent.
- Read short, plain tips based on your own care.
- Move at your own pace. Clear screens, one thing at a time.
